How to use the Dental & Vision Discount Plan

Keep your membership card with you at all times to easily present it at participating providers. You must present your membership card at the time of service to receive a discount.

Use the PROVIDER SEARCH tool to quickly and easily locate and verify participating providers in your area.

You are responsible for paying the provider the entire discounted fee at the time the service is provided.

When will my plan be effective?
During enrollment, you will be given the option to choose your effective date. You may choose either the first of the current month or the first of the next month.

No, this is not an insurance plan. You will pay for services at the discounted price at the time services are received.

How does this plan work?
After signing up for the Dental & Vision Discount Plan, a welcome letter and member ID cards will be mailed to your address. To search for participating providers online, click here.

Can I use my membership card when I travel away from home?
Yes, your membership card can be used at any participating provider in the United States. All you need is your member ID number to receive your savings.
